3. SMS Communications

By providing your mobile phone number, you consent to receive SMS messages from Sunrise Family Clinic including appointment reminders, intake form links, and clinic notifications. Message and data rates may apply. Message frequency varies. Reply STOP to opt out at any time. Reply HELP for help. We do not share your mobile number with third parties for marketing purposes.

4. HIPAA Compliance

We are committed to protecting your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). We implement appropriate technical, administrative, and physical safeguards to protect Protected Health Information (PHI). We do not sell or rent your health information to third parties.

We do not sell, trade, or rent your personal information to third parties. We may share information with healthcare providers involved in your care, insurance companies for claims processing, and service providers who assist in our operations under Business Associate Agreements (BAA). We retain patient records and related documentation for a minimum of 6 years as required by HIPAA regulations, or longer if required by applicable state law. SMS logs are retained for 90 days.
